Application Checklist

  • Graduate Application Form should be mailed to the Office of Admissions and Records.
  • Graduate Assistantship Form and Tuition Waiver Forms should be mailed to Graduate Secretary.
  • Applications for all graduate prgrams must include 3 Recommendation Letters and the Student Information form. These forms should be mailed to the Graduate Secretary.
